Refund/Player Drop Forms

Region 21 Player Drop and Refund Policy

Players that drop from our league may receive a refund per the following guidelines and procedure:
 
 


Guidelines:

1. Any player who withdraws prior to participating in the first game or practice of the season shall be entitled to a full refund of their registration fee, provided that the player/parent submits a request for refund by completing the form below and returns the unused uniform.

2. Any player who withdraws after participating in the first practice, but prior to the first game of the season shall be entitled to a refund of the registration fee, minus AYSO National Registration fees for insurance, provided that the player submits a request for refund by completing the form below and returns the unused uniform.

3. Any player who withdraws after participating in the first game of the season or beyond shall not be entitled to a refund of the registration fee.


Procedure:

 Parent:
The parent requesting the drop must obtain a Player Drop Form, fill out the
“Parent” section, sign, and return to coach. Form can be obtained from the team coach or Region 21 Registrar.

Coach:
The team coach must send or hand delivers the Player Drop Form with the
White copy of the Player Registration Form to their Division Coordinator.

Coordinator:
The Division Coordinator must insure that the form is completely filled out, then send or hand delivers a copy of the Drop Form with the White copy of the Player Registration Form to the Region 21 Registrar.

 

The Registrar will notify the Region 21 Treasurer
of the amount and person to send the refund to.
